La maladie d’Alzheimer est fréquente. Elle concerne principalement, mais non exclusivement des personnes âgées et très âgées. Elle s’accompagne de troubles mnésiques et du jugement. La sémiotique permet d’approcher les raisons de la perte du sens dans l’environnement matériel ethumain du malade. Elle pointe et qualifie des efforts déployés par le patient pour qu’il reste inscrit dans un monde signifiant pour lui. Elle permet enfin de trouver de nouvelles pistes pour asseoir le soin des patients déments. L’approche sémiotique a été conduite d’une part à partir de deux études, l’une sur la narrativité d’un texte mémorisé chez des patients déments, l’autre sur la dissolution des limites dans la relation du malade à son aidant à domicile, et d’autre part à partir de l’analyse de cas cliniques. De nouveaux parcours ont été décrits dans l’histoire naturelle du patient selon l’évolution de l’espacetensif et selon ses formes de vie. Des ruptures dans la continuité de ces parcours apparaissent, précipitant une situation irréversible. Une prévention de celles-ci a été proposée. Les altérations du couplage entre la présence du monde à l’intimité psychique du patient et sa présence au monde ontdes conséquences sur le sens engagé à la fois dans ses agirs et dans ses relations interpersonnelles. La relation humaine réciproque et l’appui sur l’imaginaire du malade sont des ressources thérapeutiques. Une ressource thérapeutique nouvelle a été proposée : l’analogon.

Alzheimer's disease is common. Mainly, but not exclusively, this disease concerns elderly and very old people. Disease is associated with memory disorders and Impairment of judgment. Semiotics allows to approach the reasons of the loss of the sense in the physical surrounding and human environmentof the patient. Semiotics points and qualifies efforts made by patients to remain in a signifying world for them. Semiotics put in perspective new ways to establish patient care. The semiotics’ approach in this thesis was conducted on one hand from two studies, one on the narrativity from a text memorizedby demented patients, the second on the dissolution of the limits in patient's relationship with their community caregiver, and on other the other hand, from clinical cases analysis. New pathways have been described in the natural history of demented patients according to the evolution of the tensivespace and according to their forms of life. Rupture in the continuity of the course of the illness appears, precipitating an irreversible situation. Prevention of this break has been proposed. The alterations of the coupling between the presence of the world in patients ‘psychic intimacy and their presences ofself in the world have consequences on the mean ing involved both in their actions and in their interpersonal relations. The reciprocal human relation and using world of the patients ‘imaginary are therapeutic resources. A new therapeutic resource has been proposed: the analogon.
