Quality certification and accreditation has been a challenge for French healthcare organizations for over a decade. Without constituting real failures, they meet however some difficulties, certain practices or tools having difficulties in being established. This article presents the results of a research, conducted within a regional university hospital, to identify the reasons for these difficulties. It seems that it is not staff adaptation or adhesion to the approach that are the most problematic, but rather the obstinacy of familiar schemas putting obstacles to the transverse approaches demanded by quality management.
