La maladie à corps de Lewy (MCL) avec troubles neurocognitifs majeurs présente deux formes cliniques : la démence à corps de Lewy (DCL) et la démence parkinsonienne. Elle se caractérise sur le plan biologique par une altération de la neurotransmission avec notamment un profond déficit en acétylcholine et un déficit en dopamine. Le déficit cholinergique est impliqué dans les troubles attentionnels majeurs et les fluctuations. Les inhibiteurs de l’acétylcholinestérase ont fait l’objet de plusieurs études en double aveugle contre placebo. Les méta-analyses montrent un bénéfice modéré sur le plan cognitif et sur certaines manifestations psychiatriques, dont les hallucinations. Un syndrome parkinsonien invalidant peut être traité par L-dopa comme dans la maladie de Parkinson, mais l’augmentation des doses est limitée par les troubles cognitifs et psycho-comportementaux. Les hallucinations peuvent nécessiter un traitement par la clozapine, les autres antipsychotiques étant contre-indiqués. Les troubles du comportement en sommeil paradoxal sont améliorés par un apport de mélatonine. À côté de ces manifestations caractéristiques de la MCL, bien d’autres sont gênantes (anxiété et dépression, hypotension orthostatique, constipation…) et sont envisagées avec les possibilités thérapeutiques plus ou moins bien documentées. À chaque fois, sont aussi étudiées les prises en charge non médicamenteuses (stimulation cognitive, kinésithérapie, orthophonie…). Enfin, une proposition de stratégie thérapeutique est suggérée à partir des preuves et de l’expérience clinique.
