Le bloc paravertébral thoracique en injection unique ou en perfusion continue à travers un cathéter, permet l’analgésie et l’anesthésie métamérique unilatérale. L’utilisation de ce bloc dans la pratique clinique est freinée par le taux d’échec et l’imprévisibilité de la diffusion métamérique qui se traduit par une analgésie inconstante. Nous avons étudié la technique, les approches, le côté, le niveau, le nombre d’injections et le modèle de cathéter comme possibles variables pouvant influencer le taux de succès et la diffusion métamérique.Nous avons modélisé le bloc paravertébral thoracique par des injections paravertébrales de produit coloré ou radio opaque chez des sujets cadavériques et analysé la diffusion par dissection anatomique et/ou imagerie.La technique avec échoguidage est associée à un taux de succès plus élevé par rapport à la technique par perte de resistance. Aucune influence sur le taux de succès ou la diffusion n’a été retrouvée par rapport à une approche échoguidée spécifique, ni au niveau d’injection et la dilatation préalable de l’espace ne semble pas faciliter la diffusion du produit marqué. La répartition du volume en deux injections séquentielles espacées permet une diffusion métamerique plus étendue par rapport à une injection unique du même volume total.Le taux de succès après pose d’un pose d’un cathéter et la diffusion du liquide ne semblent pas être influencés par le modèle de cathéter, l’approche échographique, le niveau ou le côté.Des études cliniques sont nécessaires pour vérifier ces données et explorer les effets indésirables.

Thoracic paravertebral block as a single injection or as a continuous infusion through a catheter, produces metameric unilateral analgesia or anesthesia. Its use is hampered in clinical practice by the failure rate and the unpredictability of metameric diffusion which results in inconstant analgesia. We studied the technique, the approaches, the side, the level, the number of injections and the catheter model as possible variables which can influence the success rate and the metameric spread.The thoracic paravertebral block was reproduced by thoracic paravertebral injections of coloured or radiocontrast solution in cadaveric subjects and spread was measured by anatomical dissection and / or imaging.The ultrasound-guided technique is associated with a greater success compared to the technique using a loss of resistance. The success rate and spread pattern were not linked to a specific ultrasound guided approach, nor to the thoracic level of injection. Prior dilation of the space does not seem to facilitate the subsequent liquid diffusion. The distribution of the volume in two separate injections allows a larger metameric diffusion compared to a single injection of the same total volume. The success rate and spread of the liquid was not influenced by the catheter model, ultrasound approach, level or the puncture side. Clinical studies are necessary to confirm these data and to explore adverse effects.
