In every woman the perinatal period is marked with contradictory desires and ambivalent feelings which little by little build a link of attachment with the child. In all these mothers, we can see the coming, whether admitted or not, of aggressive thoughts linked to the necessary infantile renunciation and the devotion indispensable for access to the maternal position. In some patients, these aggressive thoughts cannot be contained, and break into the psyche, causing disorganization capable of jeopardizing thymic equilibrium, access to maternality and even the establishment of the mother/child link. Referring to clinical observation we shall show an example of care in one of these complex risk situations: we shall then put forward some leads for understanding this phenomenon, and suggest a therapeutic approach in line with writings and our clinical practice.
