Dans l’instant de dire, le patient met en récit son symptôme à partir de ce qui l’amène à consulter. Du fait même de l’offre d’écoute du praticien ordonnée par la libre association, la plainte du patient se construit en roman de la maladie. Ce roman de la maladie s’avère exclusif, « complémentaire » au sens épistémologique, du symptôme et de la maladie tels que la médecine scientifique les objective. En cela la notion de psycho-somatique peut, comme à ses débuts, se réécrire avec un trait d’union tout autant que de séparation et favoriser la réconciliation de la psychanalyse et de la médecine. Dans notre pratique, le temps et la temporalité ont plus à voir avec l’instant qu’avec la durée et l’instant se définit comme temps humain tragique de notre post-modernité. Privilégier l’instant dans la clinique, c’est encore là aussi renouveler la réflexion épistémologique sur la place de la psychanalyse au sein de notre médecine technoscientifique. De là, nous aborderons plus spécifiquement la manière dont le corps peut se dire lorsque les traces des blessures passées persistent, se trouvent fixées dans la mémoire vive et aliènent le sujet dans un réseau de signifiants qui ne demandent qu’à devenir jeu de signifiants à l’occasion d’un instant de dire.

In the instant of saying, patients put into story form their symptoms deriving from the condition brought them to consult. Due to the very fact of the offer of listening by the practitioner, as dictated by free association, the patient’s complaint is built up as the story of the illness. This illness story proves to be exclusive, «complementary» according to the epistemological meaning, of the symptom and the sickness as scientific medicine objectifies them. In this the psycho-somatic concept can, as at its beginnings, be re-written with a hyphen or even separated and foster the reconciliation between psychoanalysis and medicine. In our practice, time and temporality have more to do with the instant than with duration and the instant is defined as tragic human time of our post-modernity. Favouring the instant in clinical treatment also means renewing epistemological thinking on the place psychoanalysis occupies within our techno-scientific medicine. Taking things further, we shall more specifically address the way the body can express itself when traces of past wounds persist, when they are inscribed in living memory and alienate the subject in a network of signifiers that seek simply to become a set of signifiers when it comes to an instant of saying.
