L’auteur souhaite montrer qu’en médecine intensive-réanimation la pulsion de vie est d’abord du registre du besoin. L’objet secourable donne les soins de première nécessité au patient pour le ramener à la vie. Dans un second temps, la clinicienne à son chevet entend son histoire. Elle est elle-même le lieu d’une reviviscence pulsionnelle puissante, d’un contre-transfert massif qui serait comme un mandat libidinal où la pulsion de vie est transfusée, brouillant les limites entre soi et l’autre. Il s’agit d’interroger, dans cette clinique de l’extrême, comment Éros peut être réintriqué au registre du besoin et de se demander quelle est la place du sexuel, même élargi, dans cet espace-là, et quelles formes peut prendre la libido dans le temps du risque de mort et de survie. L’auteur propose d’interroger cette reterritorialisation du pulsionnel, dans la fonction soignante plurielle, dans le temps de la rencontre psychique au lit du patient et dans le champ du transfert.
The author hopes to show that in medical intensive care and reanimation the life drive belongs first and foremost to the register of need. The helping object gives basic life-saving healthcare to the patient to bring him/her back to life. Later on, the clinician at the patient’s bedside hears his/her story. This is itself the occasion of a powerful drive reawakening, of a massive countertransference, which is like a libidinal mandate where the life drive is transfused, blurring the boundaries between self and other. In this extreme clinical situation, it is a question of asking how Eros can be fused once again with the register of need and of asking ourselves what the place of the sexual, even in an enlarged sense, is in this space, and what forms the libido can take in a period of risk of death and survival. The author examines this reterritorialization of instinctual drive activity in the plural caring function, in the moment of the psychic encounter at the patient’s bedside and in the field of the transference.
La autora quiere mostrar que en medicina intensiva-reanimación la pulsión de vida emana ante todo de la necesidad. El objeto auxilio brinda cuidados de primera necesidad al paciente para reanimarlo a la vida. Luego, el clínico responsable entiende su historia. Constituye el espacio de un revivir pulsional poderoso, de una contratransferencia masiva suerte de mandato libidinal en el que la pulsión de vida es transmitida, borrando los límites entre sí y el otro. Se trata de interrogar, en esta clínica de lo extremo, como Eros puede ser refusionado bajo la necesidad y de preguntarse qué sitio ocupa lo sexual, incluso ampliado, en ese ámbito, y qué formas puede tomar la líbido en el tiempo de riesgo de muerte y de supervivencia. La autora propone interrogar la reterritorialización de lo pulsional, en la función sanitaria plural, en el tiempo del encuentro psíquico en la cama del paciente y en el campo de la transferencia.
