The evolution of health policy is best understood in the context of the transformation of the welfare state. Existing institutional arrangements and path dependence do not prevent serious reforms as the French case testifies. The French reforms include a retrenchment of health insurance which has obliged the state to develop targeting policies in order to prevent an increase in inequality of health provision. The genealogy of these transformations of health provision in the welfare state is the subject of this volume.

L’évolution de la politique de santé s’inscrit dans le champ des transformations de l’Etat social. La dépendance au sentier et les configurations institutionnelles existantes n’empêchent pas une réforme profonde comme en témoigne le cas français. Cette réforme organise un retrait de l’assurance maladie qui oblige l’Etat social à développer des politiques de ciblage pour ne pas accroître les inégalités sociales de santé. C’est à une généalogie du nouvel Etat social en santé qu’il convient de contribuer.
