Concepções de natureza, paradigmas em saúde e racionalidades médicas

Neste artigo, baseado em uma revisão crítica da literatura, desenvolvemos uma breve reflexão sobre diferentes concepções de natureza e suas consequências na terapêutica ao longo da história. Ressaltamos a presença de dois paradigmas em saúde, o vitalista e o biomédico, que embasam distintos sistemas médicos complexos, ou racionalidades médicas coexistentes na cultura atual. Trabalhamos com a hipótese de que há uma retomada e ressignificação do paradigma vitalista na atualidade, como portador do ideário de contracultura. Apresentamos, então, a categoria de análise “racionalidade médica” e o projeto de pesquisa de mesmo nome, que surgiram em 1992, no campo da Saúde Coletiva, área das Ciências Sociais e Humanas em Saúde. Destacamos seus principais desdobramentos e contribuições na prática profissional e política em favor da abertura ao intercâmbio solidário de diferentes saberes e práticas terapêuticas em saúde.

In this article, based on a critical review of the literature, we develop a brief reflection on different conceptions of nature and its consequences in therapy throughout history. We highlight two paradigms in health, biomedical and vitalist, which underlie different complex medical systems, or coexisting medical rationalities in the current culture, and understand that there is resumption and reframing of the vitalist paradigm nowadays, as the bearer of the ideals of the counterculture. Then we present the analysis category “medical rationality” and the research project of the same name, which appeared in 1992 in the field of Collective Health, Social Sciences and Humanities in Health. We emphasize its main developments and contributions in the professional and political practices in favor of opening the exchange of different supportive knowledge and therapeutic practices in health.
