Les auteurs réexaminent le rôle que tient la démarche diagnostique dans la fondation freudienne. Ils dégagent les arêtes épistémologiques de la question du diagnostic dans la psychanalyse. La nécessité clinique de se sortir du chaos de la nosographie de l’époque se traduit dans la psychanalyse naissante par un travail d’identification et de différenciation des tableaux cliniques depuis les mécanismes qui les sous-tendent. Tant la tension entre l’indéterminé du cas individuel et la surdétermination du type nosographique que l’inéluctable « mixité » de tout tableau clinique guident une démarche psychopathologique où l’investigation diagnostique se noue nécessairement à l’action thérapeutique. Dans cette approche, le traitement acquiert la dimension d’être la seule méthode diagnostique efficiente.

The authors re-examines the key role played in the Freudian method by the diagnostic approach. They intend to identify the epistemological specificities of diagnosis in psychoanalytic context. The clinical requirement to escape from the “chaos” of nosography of his time leads Freud, in the beginning of psychoanalysis, to investigate, identify and differentiate several clinical pictures as regard to their underlying mechanisms. The tension between the idiosyncrasy of the individual case and the overdetermination of the nosographic type, so as the inevitable ‘mixing’ of all clinical pictures in the clinical practice drive a psychopathological approach into which the diagnostic investigation and the therapeutic process are closely associated. In such approach, the psychical treatment seems to be the only efficient diagnostic method.
