Resumé La pratique des entretiens psychologiques est devenue courante en soins palliatifs. Elle confronte les psychologues à des transferts intenses et condensés dans le temps. Rédiger des comptes rendus est habituel pour élaborer à la fois son contre-transfert et la détresse des patients. Aucune échelle ne facilite actuellement cette analyse. Un collège régional de psychologues en soins palliatifs a réalisé un livret de recueil de leurs impressions cliniques. Cette étude originale constituait la première phase d’une recherche doctorale « L’identité à l’épreuve de la maladie létale », débutée en 2009. 1) Objectif : disposer d’un outil d’analyse permettant une mise en commun des données à des fins de recherche. 2) Méthodologie : l’investigateur principal a sélectionné 30 concepts de psychologie clinique adaptés aux soins palliatifs pour évaluer les processus psychiques des patients altérés par la pathologie, le contre-transfert des psychologues et les fonctions des entretiens. 15 psychologues de 12 équipes de soins palliatifs (représentatives des différents modèles français) l’ont testé pendant un mois puis ont rempli des rapports d’utilisation. 3) Résultats : les psychologues se sont appropriés le livret qui était étayant et adapté à leur pratique. 4) Discussion : des conditions d’utilisation se sont imposées, notamment la création d’un espace inter-subjectif. 5) Conclusion : le livret est pertinent en soins palliatifs. Les psychologues l’utilisent actuellement pour tous leurs patients. Au terme d’une année d’utilisation, nous disposerons de suffisamment de données pour comprendre les liens entre la détresse des patients, leurs ruptures identitaires et l’intensité des contre-transferts des psychologues.

Development of a psychic processes inventory in French palliative carePsychologists using psychological interview in palliative are confronted with intense emotional and transference processes. Unfortunately there is no available French instrument to ease the reporting of such experiences. A regional palliative care psychologist forum developed a standardized booklet to document clinical impressions in palliative care. This study was the first step of a PhD research on « the person’s identity facing a lethal disease », beginning 2009. Objective : To dispose of a standardized tool to gather clinical research data. Method: the main investigator selected 30 clinic psychology concepts in palliative care assessing the patient’s psychic processes in the context of a terminal illness, the psychologist’s internal emotional status, and the role of psychological interviews. Fifteen psychologists from 12 palliative care teams representative of the various French models, tested the tool during one month and filled out feedback reports. Results: This tool was considered by psychologists as interesting and useful to support their practice. Guidelines regarding the passation of the interview and the use of the booklet were delineated. Discussion/ Conclusion: in keeping with the lack of current appropriate French instruments, our booklet appeared to be relevant to palliative care. Palliative care psychologists in our Region are currently using it to follow their own patients. The second step of our research will consist of a one-year clinical trial of the booklet. We expect to gather enough data to understand the links between the patients’ distress, their identity crisis and the treating psychologists’ own internal emotional status.
