This study seeks to evaluate psychotherapeutic changes using the Rorschach according to the three levels of communication defined by Fonagy and colleagues: epistemic trust, mentalization, and resocialization. Two groups of patients were compared: 39 clinically improved patients and 30 clinically unimproved patients according to the therapists’ and the patients’ own opinions. The Rorschach was administered at baseline (t1) and at least one year later (t2). Five of the six indices drawn from the Rorschach to assess the evolution of the three levels of communication show significant differences between clinically improved and clinically unimproved patients. The interest of these indices for the understanding of the psychotherapeutic process and the study of the interactions between the three levels of communication is discussed.
