Approche psychopathologique de l’enfant en nutrition parentérale prolongée ou transplanté de l’intestin

Ce travail de recherche, rendu possible par une étroite collaboration entre le service de Pédopsychiatrie et le Service de Gastro-entérologie de l’Hôpital Necker-Enfants Malades, propose d’étudier l’impact de la nutrition parentérale prolongée (NPP) et de la greffe intestinale sur le psychisme de l’enfant qui en bénéficie. L’objectif est d’étudier chez ces enfants le vécu psychologique, l’image du corps, la représentation psychique de l’organe malade et du greffon ; et chez les parents, le vécu psychologique. Les outils utilisés sont les dessins du bonhomme et du contenu intestinal avec l’observation clinique (enfants) ; des entretiens semi-structurés (vécu des parents). L’analyse qualitative des entretiens a permis de dégager des thèmes principaux (représentation de la NPP, répercussion sur la relation parents/enfants, représentation de la greffe) et des sous-thèmes. L’analyse des dessins et l’observation clinique ont permis de retrouver une image du corps perturbée. L’hypothèse que l’investissement psychique du greffon intestinal (archaïque/ génitalisé) peut être mis en évidence par les caractéristiques du dessin du contenu intestinal est confirmée.

This research study, made possible by a close collaboration between the pediatric psychiatry and gastroenterological services of the Hôpital Necker-Enfants Malades, studies the impact of long-term parenteral nutrition (PN) and intestinal transplant on the psychic mechanism of the child being treated. The objective is to study the psychological experience of these children, their body image, their psychic representation of the ailing organ, and the graft, and the psychological experience of the parents. The tools used are the Draw-a-Person test and drawing the contents of the intestine, with clinical observation (children) and semi-structured interviews (parents’experience). Qualitative analysis of the interviews has allowed us to identify some major themes (representation of long-term PN, repercussions on the parent-child relationship, representation of the transplant) and sub-themes. Analysis of the drawings and clinical observation has allowed us to discover a disturbed body image. Our hypothesis that the psychic investment of the intestinal graft (archaic/genitalized) can be brought to light through the characteristics of the drawing of the intestinal content is confirmed.

El impacto de la alimentación parenteral prolongada es el objeto de esta investigación que ha sido posible gracias a la estrecha colaboración entre el Servicio de Pedo-psiquiatría y el Servicio de Gastroenterología del Hospital Necker-Enfants Malades. Se trata de estudiar el impacto de la alimentación parenteral prolongada y de y del trasplante intestinal en el psiquismo del niño. El objetivo es estudiar las vivencias psicológicas de esos niños, la imagen corporal, la representación psíquica del órgano enfermo y del implante; y la vivencia psicológica de los padres. El dibujo del hombre y del contenido intestinal son los instrumentos utilizados y la observación clínica (de niños) y las entrevistas más o menos estructuradas (vivencias de los padres. Gracias al análisis cualitativo de las entrevistas se ha podido destacar los temas principales (Representación de la alimentación parenteral prolongada APP, repercusión en la relación padres/hijo, y representación del implante) y de los sistemas. Gracias al análisis de los dibujos y la observación clínica hemos podido localizar la imagen de un cuerpo perturbado. La hipótesis de la investidura psíquica del implante intestinal (generalizado y arcaico) puede evidenciarse mediante las características del dibujo del contenido intestinal se confirma.
