Traitement psychanalytique d’un épisode confusionnel survenu à la suite d’une intervention chirurgicale mutilante. Les réactions violentes de la patiente plongent l’équipe soignante dans le désarroi. L’intervention décalée de l’analyste dans le transfert entraîne simultanément un apaisement et une prise de parole. Ensuite, son écoute permettra un véritable travail psychique. Pour l’analyste, c’est seulement dans l’après-coup – et au prix pour lui aussi d’un véritable travail psychique – que pourra se produire l’élaboration clinico-théorique du cas, ainsi que son écriture.

Psychoanalytical treatment of a delirious episode following a mutilative surgical intervention : the violent reactions of the patient plunge the medical team into a state of confusion. An indirect approach to the delirium on the part of the analyst in the transference resulted in simultaneously calming the patient and her talking and revealing her history. This was then followed by actual psychoanalytical work. For the analyst it is only after crisis intervention (après-coup) – followed by actual psycho-analytic working-through – that a clinico-theoretical evaluation of the case in both analytical and written form can be made.
