Les thérapies orales en 2021 dans les leucémies aiguës myéloïdes : un travail d’équipe ! L’expérience toulousaine

En ce début d’année 2021, en France, nous avons la possibilité d’utiliser des thérapies orales pour le traitement des leucémies aiguës myéloïdes, soit en monothérapie en situation de rechute ou de maladie réfractaire (enasidenib, ivosidenib, gilteritinib) ou alors en association avec l’azacitidine en première ligne chez des patients non éligibles pour la chimiothérapie intensive en ce qui concerne le vénétoclax. Nous apprenons à manier ces nouveaux médicaments qui ont chacun leur profil de tolérance, et nous avons à nous habituer à cette situation où des patients fragiles sont souvent à leur domicile. Comment concilier nouveaux médicaments, schémas complexes, polymédication, transfusions, gestion des complications et ambulatoire ? Un véritable travail d’équipe permet de relever ce défi ! Nous exposons ici des pistes issues des recommandations de la littérature, notre retour d’expérience, l’interview d’une IDE coordinatrice spécialisée en hématologie et le point de vue d’un pharmacien hospitalier.

In the beginning of 2021 in France, oral therapies are available for the treatment of acute myeloid leukemia, either as monotherapy in relapsed or refactory diseases (enasidenib, ivosidenib, gilteritinib) or in combination with azacitidine for patients unfit for intensive chemotherapy at first line as regards venetoclax. We are learning to handle with these new drugs that have each their own toxicity profile, and we have to get used to this new situation, where frail patients are often at home. How can we conciliate new drugs, complex regimens, polymedication, transfusions, complications and outpatient? A true team work can take on this challenge! Here we present recommendations from the literature, the experience of our center, the interview of a nurse specialized in hematology and the point of view of an hospital pharmacist.
