Dans cet article, nous exposons deux modes d’intervention destinées à permettre au patient de mieux réguler certaines attitudes autodestructrices. Nous les utilisons à l’occasion et à temps partiel, à partir de 15-16 ans, dans des psychothérapies plus vastes et plus traditionnelles, où l’écoute joue un rôle majeur. La première consiste pour le patient à combattre directement, via exercices appropriés, un symptôme autodestructeur : par exemple, une rumination anxieuse chez un grand stressé. La seconde consiste en un défi subtil qui convainc le patient de l’absurdité de son symptôme, par exemple, via une utilisation judicieuse de l’humour. Toutes deux visent à remettre le patient en position d’acteur en lui faisant prendre conscience de son potentiel pour réguler ces modes de pensées et comportements contre-productifs, en déclenchant une réaction de saine rébellion contre eux et en réactivant sa créativité. Leurs indications sont analysées en fonction de différents paramètres propres au patient et au diagnostic clinique.

In this article, we present two psychotherapeutic interventions aimed at helping the patient to regulate certain self-defeating behaviors. We use them occasionally and on a part-time basis with patients older than 15-16 years, including them in larger and more traditional therapies where attentive listening plays a major role. The first intervention is to encourage the patient to overtly fight, with appropriate exercises, self-defeating symptoms such as anxious ruminations. The second is to subtly challenge the patient to acknowledge the absurdity of his symptoms, for example through judicious use of humor. Both aim to put the patient back in the position of actor, by making him aware of his potential to regulate these counter-productive ways of thinking and behaviors, by triggering a reaction of healthy rebellion against them and by reactivating his creativity. The indications are discussed according to various parameters specific to the patient and clinical diagnosis.
