— Un travail psychanalytique à plusieurs est indiqué pour traiter cette forme particulière de transfert qui survient régulièrement dans le traitement institutionnel de jeunes patients souffrant de décompensation psychotique ou de troubles graves du comportement. L’ aliénation y est telle, en effet, que le déterminisme de répétition tend à se trouver induit dans l’autre – les thérapeutes, en l’occurrence – en l’absence de fantasme constitué chez le patient lui-même. Un cas comme celui d’Angel peut donner quelque idée de ce phénomène amenant plusieurs soignants à constater qu’ils endossent des vécus incompatibles, réactualisant sans doute des perceptions premières du patient, en défaut d’image mentale. La restitution (Freud, 1937) de ces données existentielles premières doit avant tout s’effectuer par leur saisie dans l’espace psychique des soignants. L’aptitude de ceux-ci à les reconnaître, les supporter et les élaborer entre eux – comme un transfert – va conditionner la possibilité pour le patient de les subjectiver.
Summary — Group psychoanalytic work is recommended to treat the particular form of transference that regularly arises in the institutional treatment of young patients suffering from psychotic collapse or serious behavioural problems. Alienation in this case is so extreme that the determinism of repetition is often induced in the other, that is to say the therapists concerned, in the absence of a constituted phantasy of the patient. The case of Angel gives us an idea of this phenomenon that leads a number of therapists to conclude that they are in the position of having to shoulder incompatible experiences that no doubt reconstitute the patient’s earliest perceptions in the absence of mental images. The restitution (Freud, 1937) of these early existential features must necessarily take place primarily via their establishment in the therapists’ psychic space. The aptitude of the latter to recognise, take on and elaborate them amongst themselves, like a transference, is a necessary condition for the patient to be able to subjectivise them.
Zusammenfassung — Die psychoanalytische Zusammenarbeit von mehreren Kollegen ist angegeben, um diese spezielle Form von Übertragung zu behandeln, welche regelmässig in der institutionellen Behandlung von jungen Patienten aufkommt, welche an psychotischer Dekompensation oder schweren Verhaltensstörungen leiden. Die Störung zieht nach sich, dass der Wiederholungsdeterminismus sich beim andern einprägt – bei den Therapeuten – aufgrund der Absenz von konstituierten Fantasmen beim Patienten. Ein Fall wie Angel kann einige Ideen über dieses Phänomen hervorrufen, indem mehrere Pfleger bemerken, dass sie unvereinbare Erlebnisse auf sich nehmen und somit Erstwahrnehmungen des Patienten aktualisieren, welchen die psychischen Bilder fehlen. Die Restitution (Freud, 1937) dieser existenziellen Ersterlebnisse muss im Raum der Pfleger erkannt werden. Die Fähigkeit des Pflegepersonals, sie zu erkennen, zu ertragen und utereinnder auszuarbeiten – wie eine Übertragung – ist die Bedingung für den Patienten, um sie sujektivieren zu können.
Resumen — Un trabajo psicoanalítico grupal es preconizado para tratar esa forma particular de transferencia que se presenta regularmente en el tratamiento institucional de pacientes jóvenes que sufren una descompensación sicótica de trastornos graves del comportamiento. La alienación es tal, que el determinismo de repetición tiende a hallarse inducido en el otro – aquí se trata de los terapeutas – ante la ausencia de fantasía en el propio paciente. Un caso como el de Angel puede proporcionar algunas ideas de dicho fenómeno que lleva a varios terapeutas a constatar que ellos absorben vivencias incompatibles, sin duda reactualizan percepciones primeras del paciente, ante la falta de imagen mental. La restitución (Freud, 1937) de dichos elementos existenciales primeros debe ante todo ser integrada en el espacio psíquico de los terapeutas. La aptitud de éstos para reconocerlos, soportarlos y elaborarlos entre ellos – como una transferencia – condicionará la posibilidad del paciente para subjetivizarlos.
Riassunto — Un lavoro psicoanalitico in gruppo è indicato al trattamento di quella forma particolare di transfert che interviene regolarmente nella cura in istituzione di giovani pazienti affetti da scompenso psicotico o da disturbi gravi del comportamento. L’alienazione è tale, infatti, che la coazione a ripetere tende ad essere indotta nell’altro – in questo caso negli psicoterapeuti – in assenza di fantasma costituito nel paziente stesso. Un caso come quello di Angel puo’ dare un’idea di questo fenomeno che porta diversi operatori a constatare come essi si addossino dei vissuti incompatibili, che riattualizzano probabilmente delle percezioni primarie del paziente in mancanza d’immagine mentale. La restituzione (Freud, 1937) di questi dati esistenziali primari deve effettuarsi anzitutto attraverso la loro individuazione nello spazio psichico del personale curante. L’attitudine degli operatori a riconoscerli, sopportarli e ad elaborarli fra loro – come un transfert – condizionerà la possibilità per il paziente di soggettivarli.
