Interprofessionnalité en santé mentale : le cas des dispositifs conventionnels et non conventionnels en Lorraine

À partir d’une recherche en psychologie et en anthropologie portant sur l’analyse des dispositifs alternatifs en santé mentale intitulée Altermental sur le territoire lorrain, cet article vise à mettre la dégradation systématique de la place de la psychothérapie dans le soin psychiatrique public en regard de l’essor des dispositifs alternatifs en santé mentale, dans un contexte où les dispositifs conventionnels et non conventionnels sont en situation de concurrence interprofessionnelle. Les analyses s’appuient sur des entretiens conduits avec 29 patients, praticiens non conventionnels et praticiens conventionnels dans cette région. L’analyse de ce matériel montre que les différents discours encouragent majoritairement la complémentarité des approches et la condamnation des parcours strictement alternatifs basés sur un rejet du biomédical. Des dérives et des tensions conduisent à une comparaison critique des deux types de dispositifs. La dégradation des conditions de pratique de la psychiatrie publique et la déchéance de la figure médicale paternaliste sont reconnues – y compris par les praticiens conventionnels – comme les paramètres systémiques dont il faut tenir compte pour analyser l’évolution de cette situation.

Based on psychological and anthropological research on the analysis of alternative mental health systems in Lorraine, entitled Altermental, this article examines the systematic degradation of the place of psychotherapy in public psychiatric care and the rise of alternative systems in mental health, in a context where conventional and non-conventional systems are in a situation of interprofessional competition. The analyses are based on interviews conducted with 29 patients, non-conventional practitioners and conventional practitioners in this region. The analysis of this material shows that the different discourses mostly encourage the complementarity of approaches and the condemnation of strictly alternative paths based on a rejection of biomedical care. The drifts and tensions lead to a critical comparison of the two types of care practices. The deterioration of the conditions of psychiatry practice and the decline of the paternalist medical figure are recognized – including by conventional practitioners – as systemic parameters that must be considered in order to analyze the evolution of this situation.
