Cette recherche a pour objectif de préciser les caractéristiques constitutives des Innovations Organisationnelles en Santé (IOS) développées dans le cadre de l’article 51. Introduit par la loi de financement de la sécurité sociale 2018, l’article 51 constitue un protocole catalyseur de l’innovation en santé, dont la particularité est d’être à l’initiative des acteurs et de déroger aux règles organisationnelles et de financements classiques. C’est ainsi un terreau fertile pour dresser un état des lieux des IOS en cours, dans le contexte français. Sur un plan méthodologique, les cahiers des charges et les avis du comité technique des projets expérimentés dans le cadre de l’article 51 sont analysés pour développer une catégorisation des IOS. Les résultats, qui proposent une analyse descriptive des projets initiés, soulignent ainsi l’existence de 4 catégories principales d’IOS dans l’article 51 : Coordination et Parcours pour une intervention pluri-professionnelle et un décloisonnement organisationnel ; Outils numériques et télémédecine à visée transformative organisationnelle ; Prescription et conciliation médicamenteuse renforcées par la communication et le développement de nouvelles relations entre acteurs ; Prévention, accès aux soins et traitement. L’analyse des conditions de fonctionnement de ces innovations, récemment expérimentées, insiste sur les attributs de chaque type d’innovations qu’il convient de prendre en compte pour atteindre la performance attendue.

This research aims to specify the constitutive characteristics of Organizational Innovations in Health (IOS) developed within the framework of article 51. Introduced by the 2018 social security financing law, article 51 constitutes a catalyst protocol innovation in health, whose particularity is to be at the initiative of the actors and to derogate from the traditional organizational and financing rules. It is thus a fertile ground for drawing up an inventory of IOS in the French context. From a methodological point of view, the specifications, and the opinions of the technical committee of the projects developed under Article 51 are analyzed to develop a categorization of IOS. The results, which offer a descriptive analysis of the projects initiated, thus underline the existence of 4 categories of IOS: Coordination and Pathways for a multi-professional intervention and organizational decompartmentalization; Digital tools and telemedicine for organizational transformation; Prescription and medication conciliation reinforced by communication and the development of new relationships between actors; Prevention, access to care and treatment. The analysis of the operating conditions of these recently tested innovations emphasizes the attributes of each type of innovation that must be considered to achieve the expected performance.
