Today, the Narrative Medicine training of health providers has three main challenges. How to overcome the limitations of individual training? How to promote the spread of Narrative Medicine? How to build narrative environments? This dissertation provides theoretical and pragmatic answers by considering the Narrative Medicine training that we have been carrying out at the Centre Hospitalier Intercommunal de Créteil (CHIC) since 2019.At first, we explain the methodology of this project based on the principles and practice of Narrative Medicine, as Rita Charon has elaborated them at Columbia University. At the CHIC, five interprofessional groups attend a two-hour Narrative Medicine workshop once a month: they read, write, and share texts and narratives of different nature (literary, clinical, artistic, and musical). At least, a «literary consultant» and a health professional provide the facilitation.In the light of experience, we developed an innovative three-phase model (Basic Workshops, Creative Workshops, Cofacilitated Workshops), and we describe it broadly. Our goal is to offer a training device that can be transferable and adaptable to other hospitals.Using an approach based on the dialogical research, as established by Arthur Frank, we provide feedback from participants to evaluate the effects of this training on professional wellbeing, narrative competence, and institutional affiliation. We don’t forget to analyze the limits of this first experience, which must be extended within CHIC and disseminated to other hospitals. Thus, we could put our device to the test and revise or correct it (if necessary).We propose, in the end, the concept of a «narrative hospital». It allows us to deconstruct the Foucauldian metaphor of the prison, offering a model of a narrative-based hospital.

À ce jour la formation des professionnel(le)s de santé à la Médecine Narrative se mesure avec trois défis principaux : comment dépasser les limites de la formation singulière ? Comment favoriser la diffusion de la Médecine Narrative ? Comment construire des milieux narratifs ? Cette thèse donne des réponses théoriques et pragmatiques à la fois, à partir de la formation menée au Centre Hospitalier Intercommunal de Créteil (CHIC) depuis janvier 2019. Après un état de l’art, nous exposons la méthodologie de ce projet, qui se fonde sur les principes et les pratiques élaborées par Rita Charon à Columbia University. Au CHIC, cinq groupes inter-professionnels et fixes se rencontrent une fois par mois pendant deux heures, pour lire, écrire et partager des narrations (littéraires, cliniques, visuelles et/ou musicales). La co-animation est assurée au moins par un binôme médico-littéraire. À la lumière de cette expérience, nous avons développé un modèle formatif novateur en trois phases (ateliers de base, ateliers de créativité, ateliers de co-animation), qui se veut transférable et adaptable à d’autres hôpitaux. En utilisant une approche basée sur la recherche dialogique – telle qu’elle a été établie par Arthur Frank –, nous analysons les résultats de cette première expérience en termes de bien-être professionnel, compétences narratives et renforcement des liens à l’intérieur de l’institution. Nous ne manquons pas d’analyser les limites de cette première expérience, qui reste à être élargie à l’intérieur du CHIC et diffusée dans d’autres hôpitaux. Ainsi, notre modèle pourra-t-il être mis à l’épreuve, revu ou corrigé (le cas échéant).Enfin, nous proposons le concept novateur d’un « hôpital narratif ». Cela nous permet de déconstruire la métaphore foucaldienne de la prison, pour offrir un modèle d’hôpital fondé sur la narration.
