La part grandissante des thérapies orales fait apparaître une mutation dans l’organisation du projet thérapeutique en cancérologie. Tandis que l’administration d’une chimiothérapie injectable est assurée par une infirmière, et le plus souvent à l’hôpital, il appartient au patient sous anticancéreux oral de prendre son traitement au domicile, en suivant les recommandations médicales qui lui sont associées. Fondée sur une approche psychosociale, cette recherche a pour objectif de saisir la manière dont les savoirs profanes, les normes et les valeurs sociales participent à l’appropriation des anticancéreux oraux par les patients, et le rôle que le médicament joue dans la symbolisation du cancer. L’étude montre le caractère indissociable de la perception des anticancéreux oraux et des représentations que les patients ont de leur maladie. Le sens conféré au suivi de ce traitement renvoie à la valorisation de l’autonomie médicamenteuse, reflétant plusieurs formes de participation au soin et plusieurs types d’attentes concernant l’implication familiale et thérapeutique de l’entourage.
Treatment experiences and cancer experiencesThe growing proportion of oral treatments has brought light to a mutation in some parts of the therapeutics project in oncology. While the administration of an intravenous chemotherapy is done by a nurse and most of the time at the hospital, it is the responsibility of the patient under oral anticancer treatment to take their medicine at home, following the given medical recommendations. Based on a psychosocial approach, this research aims at grasping the way in which layman’s knowledge, norms and social values participate in the appropriation of the oral anticancer treatment by the patients, and the role the medicine plays in the symbolisation of the cancer. The study shows the inseparable nature of the oral anticancer treatment experience with the representation that the patients have of their disease. The meaning bestowed upon the treatment monitoring shows the valuing of the patient’s medicinal autonomy, which reflects several forms of participation in care and expectations towards the implications of the family and therapeutics circles.
El aumento de las terapias administradas por vía oral da cuenta de una mutación en la organización del proyecto terapéutico oncológico. Mientras que la quimioterapia inyectable es administrada por las enfermeras, generalmente en el hospital, el paciente bajo tratamiento anticancerígeno oral debe tomar su medicación en su domicilio, siguiendo las recomendaciones médicas pertinentes. Basada en un enfoque psicosocial, esta investigación busca determinar la manera en que el saber profano, las normas y los valores sociales participan de la apropiación de los anticancerígenos orales por parte de los pacientes, identificando el papel del tratamiento médico en la simbolización del cáncer. Este estudio muestra la existencia de un vínculo indisociable entre el tratamiento por vía oral y la representación que los pacientes tienen de su enfermedad. Así, el sentido que tiene el seguimiento de este tratamiento remite a la valorización de la autonomía medicamentosa, tomando en cuenta distintas formas de participación en los cuidados y varios tipos de expectativas relativas a la implicación del entorno familiar y terapéutico del paciente.
