Le parcours de soins et son financement sont abordés uniquement dans la perspective d’un financement approprié, question posée, dans la période actuelle et mettant en exergue les actions à mener sans coûts et celles pouvant nécessiter une vraie décision politique et/ou un investissement initial.L’approche tient aussi à faire ressortir l’implication possible du secteur public dans la démarche mais qui n’est pas une limite à une réflexion plus large.Le parcours de soins est une approche de terrain dans son élaboration comme dans son implication. Il est dés lors important de caractériser différentes présentations tumorales qui vont conditionner à la fois la prise en charge et ensuite impacter le financement optimal.Il peut être ainsi considérer 4 types de présentations (figure 1) : les plus évidentes : – les tumeurs fréquentes à présentation standard et – les tumeurs rares. Mais il est indispensable d’identifier 2 autres types de présentation à impact sur la prise en charge des patients qui pourraient être optimisée et qui imposent une structuration et/ou une coordination ayant dans tous les cas un impact sur le coût de la prise en charge et donc son financement : – les tumeurs fréquentes mais à présentation non standard et – les tumeurs métastatiques au-delà des premières lignes standard de traitements médicaux. La répartition des différentes populations de patients diffèrent bien évidemment selon les établissements.

The coordinated healthcare circuit and its financing are only approached with the prospect of an appropriate financing for the current period and highlights the actions to be led without costs and those being able to require a real political decision and/or an initial investment.The approach is also to highlight the potential implication of the public healthcare system without any limitation for a larger reflection.The coordinated healthcare circuit is a based on a field experience for its elaboration and its application. It is important to characterize various tumor presentations which condition the healthcare organization and the optimal financing. Therefore 4 types of tumor presentation may be considered : the most obvious : – the frequent tumors in a “standard” presentation and - the rare tumors. But it is essential to identify 2 other presentation clearly impacting the management of patients which could be optimized and impose a structuring and/or a coordination having in each case an impact on the cost of care and thus its financing : – the frequent tumors but with a “non standard” presentation and – the metastatic tumors beyond the first standard lines of medical treatments. The distribution of the various populations of patients differs naturally according to establishments.
