Winnicott relie le sentiment de continuité de l’existence aux soins parentaux et, en particulier au holding maternel qui permet à l’enfant de passer de la non-intégration de sa personnalité à l’intégration à travers des stades de dépendance d’abord absolue, puis relative, vers une indépendance de plus en plus grande. L’auteur suggère que ce processus peut se comprendre en termes de stabilité structurelle permettant à l’enfant de construire un monde intérieur et d’acquérir une identité. Il illustre cette hypothèse par le cas d’un bébé à risque autistique traité par des entretiens psychothérapeutiques mère-enfant.
